This is sample code. This is Text Only (Words Only) Captcha. You Can add a background image if you want and remove the borders of the input html tag with id 'mainCaptcha'I hope this answers your question.

Most of the javascript enabled captcha are easy pass for bots. Hence they do not serve the purpose of using captcha.
I suggest using powerful Google reCAPTCHA:
